Social Determinants of Health Roundtable 2: Building Relationships with Community Partners to Address Social Determinants of Health

Health care extends beyond care settings – into homes and neighborhoods. Community partnerships are critical to addressing public health needs and addressing nonclinical factors that influence health in their neighborhoods. But building such coalitions can be difficult to achieve. Participants will engage in a rich discussion with subject matter experts (SMEs) and health center staff at all levels to identify and share promising approaches, challenges, and successful strategies for building relationships with community partners to address the Social Determinants of Health (SDoH).  Participants will agree on a specific topic(s) to discuss and debate, and each member of the discussion is treated as a peer.
