Collaborative Care Model: A Proven Approach for Integrating Physical and Behavioral Health Care

The Collaborative Care Model is one approach to integration in which primary care providers, care managers, and psychiatric/behavioral health consultants work together to provide care and monitor patients’ progress. It has been shown to be both clinically effective and cost-effective for a variety of mental health and cooccurring conditions. In this webinar, participants will learn the elements of this model including team-based approach, care planning and service delivery.
