It is important for primary care and behavioral health professionals to be aware of the unique needs of transition aged-youth and how to best serve and support them due to their danger of falling through the gaps when aging out of services. This CoP will provide resources and approaches that can be used within community health center settings to build a network of primary and behavioral health care services and recovery supports for young adults that are transitioning out of child-serving systems and/or into adult-serving systems. Facilitators: Candice Russell, BS; Katie Crowley, BS 

Candice Russell, BS, is a senior program associate at Advocates for Human Potential (AHP). Ms. Russell has 10 years of experience in providing equity-driven Training and Technical Assistance (TTA) to behavioral health entities that are developing capacity and workforce aimed at treating and preventing mental health and addiction disorders. Ms. Russell’s passions and areas of subject matter expertise include increasing access to care for adults, people experiencing homelessness, adolescents and transition-age youth (TAY), and pregnant and postpartum women (PPW), particularly from BIPOC communities. 

Katie Crowley, BS, is a program associate at AHP. Ms. Crowley has a background in policy development and is skilled in health promotion, helping skills in the healthcare field, and customer service.

Earn 1.5 continuing education credits from The Association for Addiction Professionals (NAADAC) and the National Board for Certified Counselors (NBCC) for each CoP session, with a maximum of 12 CEs for participation in all eight sessions.
